About the RTI Act, 2005

The Right to Information (RTI) Act, 2005 empowers every citizen of India to seek information from public authorities, thereby promoting transparency, accountability, and good governance. District Headquarters Hospital (DHH), Balasore, being a Government Health Institution under the Health & Family Welfare Department, Government of Odisha, provides information in accordance with the provisions of the RTI Act, 2005.

Objectives

  • To ensure transparency and accountability in hospital administration.

  • To provide citizens access to information held by the hospital.

  • To promote responsible governance and public participation.

  • To improve public trust through openness and timely disclosure of information.

Information Available

Citizens may seek information relating to:

  • Hospital administration and organizational structure.

  • Services available in OPD, IPD and Emergency.

  • Citizen Charter and patient welfare services.

  • Budget, expenditure and procurement records.

  • Recruitment, sanctioned posts and staffing (as per records).

  • Hospital policies, circulars, office orders and guidelines.

  • Biomedical equipment and infrastructure records.

  • Public health programmes implemented by the hospital.

  • Quality initiatives such as NQAS, Kayakalp, LaQshya, MusQan and other approved programmes.

  • Any other information available in hospital records that is not exempt under the RTI Act.

Information Exempt from Disclosure

Information may be denied under the exemptions specified in Section 8 and Section 9 of the RTI Act, including:

  • Personal medical records of patients.

  • Information affecting privacy or confidentiality.

  • Information prohibited by law or court order.

  • Information that may endanger life or security.

  • Fiduciary or commercially confidential information protected under the Act.

How to Apply

  1. Submit a written RTI application addressed to the State Public Information Officer (SPIO), DHH Balasore.

  2. Clearly specify the information required.

  3. Attach the prescribed application fee as per Government of Odisha RTI Rules (if applicable).

  4. Mention your name and complete postal address for correspondence.

Time Limit

  • Information shall ordinarily be provided within 30 days from the date of receipt of the application.

  • Where the information concerns the life or liberty of a person, it shall be provided within 48 hours.

  • If the application is transferred to another public authority, the transfer should be made within 5 days as provided under the Act.

Appeal Mechanism

If the applicant is not satisfied with the reply or does not receive information within the prescribed time:

  • First Appeal: To the First Appellate Authority (FAA) of DHH Balasore within 30 days.

  • Second Appeal: To the Odisha Information Commission, Bhubaneswar, as per the provisions of the RTI Act.
